Please call during office hours to make an appointment. We cannot make appointments via email.

All fees are cash only. No insurance can be accepted or filed. Our physicians CANNOT be your primary care physician. 

The services listed on this page are available to all Palos Township residents.

In order to receive services at Palos Township Health Service, the following is required:

TWO (2) forms of ID with the current, correct Palos Township address. One form must be a current State Drivers License or State ID with the correct name and correct address.

Other acceptable forms of ID are:

• Utility bill
• Automobile insurance card or statement
• Statement from Medicaid or other Insurance
• Credit card or bank statement
